Said puppy has been examined and routine vaccinations have been given by a licensed veterinarian at Harmony Pet Care, 1208 Dolphin Ct. Waukesha, WI 53186, (262) 446-2273.

Vaccination and routine medical care schedule is as follows:

___________________Dewclaws removed and puppy examined for congenital defects.

1st visit on_____________at 8 weeks of age.

1 of 3 DHPP (Distemper, Hepatitis,  Parainfluenza, Parvo) was given.

Fecal examination was done. Results were_____________________.

Dewormed with Drontal Plus.

Heartworm Prevention (                       ) was given.

2nd visit on_____________at 12 weeks of age.

2 of 3 DHLPP (Distemper, Hepatitis, Leptospirosis,  Parainfluenza, Parvo) was given.

3rd visit on_____________at 16 weeks of age.

3 of 3 DHLPP (Distemper, Hepatitis, Leptospirosis,  Parainfluenza, Parvo) was given.

Seller will have routine vaccinations done accordingly to schedule if said puppy is still in seller’s possession.

Buyer further agrees to maintain the dog at all times in good health with his/her veterinarian’s program for annual examinations, routine vaccinations, heartworm blood tests, monthly heartworm preventative, spay/neuter, proper feeding, ect.

Buyer will be responsible for all future veterinary costs of said puppy upon leaving our care.

It is understood that all vaccinations, dewormings and details above are to be done by a licensed veterinarian. If you do the vaccinations and give medical care yourself this contract is null in void.

Feeding

Puppy is currently being fed Purina Pro Plan Sport  food.  I recommend feeding Purina Pro Plan, Science Diet, Iams or Eukanuba products. Puppy should be fed puppy food for at least 8 months to 14 months of age. Your puppy’s activity level and what your pup maybe doing at the time (hunting, training ect.) reflects on what and how much you should be feeding. At 8 months to 14 months of age you can consider switching to a  adult food following your Veterinarians recommendations.
